Welcome to the Commonwealth Community Care Patient Portal

The HealthPortal is a highly secure, web-based portal where you can access a portion of your health record with Commonwealth Community Care. In the HealthPortal, you can view:

  • Summaries of visits with your care team
  • Some test results
  • A list of your current medications
  • Current medical conditions and allergies


Please take advantage of this opportunity to more easily monitor and track your health care.

 

Here is how to set up your account in the HealthPortal

Make sure you have a working email address.
 
Then, call the HealthPortal Support line at 855-854-6055 (8 a.m. – 5 p.m., Monday - Friday) or email healthportal@commonwealthcare.org. Be ready to verify your identity by providing your date of birth, phone number, and/or Social Security number. You will also need to provide your email address, which will be your user ID.
 
After the support line has activated your account, you will receive an email with a link and a temporary password. When you log in, you will need to update your personal password to access your HealthPortal.
 
Be sure to bookmark the link and record your user name and password so you can view your information whenever you want.
 
Once you have registered, you can log in any time. When you have a visit with a CCC clinician, a visit summary will be is available in your HealthPortal within four days.
