    Commonwealth Community Care physicians are internal medicine and disability specialists who provide regular examinations, home visits when needed, management and coordination of care in emergencies and in the hospital, consultation with specialists, and medical consultation to the primary care team.

    • Nurse Practitioners / Physician Assistants
      Nurse Practitioners and Physician Assistants are your primary care clinicians and provide routine home or office visits for ongoing health issues, prompt intervention for new symptoms or problems, preventive health care and education, support and advocacy. The NPs or PAs coordinate all primary care and community services, and consultations with specialists.

    • Behavioral Health / Social Work Support
      The behavioral health team includes behavioral health specialists and social workers. The team collaborates with our primary care clinicians to address the emotional and behavioral health issues confronting patients. Social workers help with housing, family issues, and access to services. Behavioral health specialists coordinate treatment for issues such as depression, trauma, and anxiety.
      • Physical / Occupational Therapists
        Physical and Occupational Therapists provide home assessments and therapy as needed including instruction to personal care attendants and other caretakers at the patient’s request, and consultation with hospital physical therapy programs, seating clinics, and durable medical equipment vendors.
        • Durable Medical Equipment Staff
          The Durable Medical Equipment team works behind the scenes to coordinate and arrange for all your home medical equipment and supplies. Their commitment to customer service and extensive product knowledge ensures that medically appropriate equipment and supplies are delivered in a timely fashion.
          • Administrative Staff
            Commonwealth Community Care’s administrative team plays a vital role in ensuring that patients receive accessible, high quality health care by working closely with each other and our clinicians. Whether coordinating specialty referrals, arranging for transportation or refilling medication requests, they are dedicated to providing individual care and attention.
