Our Approach to Care
At Commonwealth Care Alliance, we’re dedicated to providing quality, compassionate care that meets the unique needs of each member. We’ve developed a special approach to care that focuses on including members and their families in planning day-to-day and future care according to their wishes and beliefs. We help members improve their quality of life and stay in their home, a community home, or other setting that they choose.

We provide members with a complete range of care, including preventive care, medical visits and treatment, dental care, behavioral health care, durable medical equipment, and social services and supports. Each member has a dedicated care manager and care team that coordinates these services wherever the member needs them, whether at their home, in a medical office, or at other locations.

Trusting, engaged relationships

  • Care teams earn members’ trust by being available day in and day out and by responding quickly to medical and social needs
  • Every member’s beliefs and preferences are valued and respected
  • Every member has a dedicated care manager to coordinate all aspects of their care plan
  • Comprehensive care planning looks out for both current and future needs


Addressing unmet needs

  • Care plans focus heavily on primary care and social services and supports
  • Care is delivered in interdisciplinary teams with clinical and nonclinical caregivers chosen to meet the member's needs
  • Care plans include behavioral care, help finding and using community resources, transportation to appointments, crisis support, care transition follow-up, and more


Transformative approach to care delivery

  • Person-centered care focuses on following members’ wishes and maintaining their independence and dignity
  • Home-based primary care for members who cannot get to a doctor’s office
  • Mobile integrated health program sends paramedics with behavioral health training on home visits when needed, reducing ER visits and hospitalizations
  • Community Crisis Stabilization Units care for members in a crisis, so they don’t have to be admitted to a hospital far from home
  • Life Choices Palliative Care program helps members receive compassionate end-of-life care according to their wishes
  • Opioid/Substance Use Disorder treatment program includes: continued engagement with members, including home visits, throughout treatment; protocol-driven evaluation and use of opioids in pain control; and increased interventions while reducing opioid use.
