Are you eligible for MassHealth Standard or for both MassHealth Standard and Medicare?
Do you live in the service area for CCA's Senior Care Options plan?

Have you been diagnosed with End-Stage Renal Disease (ESRD)?
If you've been diagnosed with this condition, you qualify for SCO if you no longer need regular dialysis or if you've had a successful kidney transplant. Unfortunately, if you're receiving regular dialysis, you're not eligible for SCO.

It's easy to enroll.

If you're interested in our Senior Care Options program and think you or a family member may qualify, complete our contact form and a CCA representative will contact you soon.
