New benefits designed to deliver patient-centered care that addresses members’ medical, behavioral health, and social needs

DETROIT — CCA Health Michigan, a leading integrated healthcare organization with proven expertise in complex care coordination, today announced new health plans for 2023. Southeast Michigan residents who are eligible for Medicare, and those eligible for both Medicare and Medicaid, can enroll in these plans during the Annual Medicare Enrollment Period, between October 15 and December 7, 2022. Depending on the plan members choose, they can pay $0 for a wide range of medical and behavioral health services and can receive extra benefits that help cover prescriptions, dental and vision care, over-the-counter health products, sneakers, gym memberships, and even food and utilities1.

We help individuals with their medical and behavioral health needs, while also addressing other key necessities—such as food1, fitness, and dental care—that make optimal, long-lasting health and wellness possible for all.”

Previously known as Reliance Healthcare, the CCA Health Michigan brand was launched in 2022 after a majority stake acquisition by Commonwealth Care Alliance® (CCA), a national, multi-.state healthcare organization with proven expertise in complex care coordination and delivery. Members of CCA Health Michigan will soon get access to CCA’s nationally recognized uncommon care® model, which was designed to help individuals with significant needs live healthier, more independent lives.

By partnering closely with our members, caregivers, network providers, and social support organizations, we aim to deliver more integrated care that improves the health of our communities.”

CCA Health Michigan offers two Medicare Advantage HMO plans, CCA Medicare Excel and CCA Medicare Ultima, designed for individuals who are eligible for Medicare. Depending on the plan, members receive many services at low or zero costs, including doctor visits, prescription drugs, hearing and vision exams, and chiropractic and podiatry care. These members also receive extra benefits that can add up to $6,575 in out-of-pocket healthcare costs savings each year, including comprehensive dental coverage and allowances for over-the-counter health products, eyewear, and hearing aids.

CCA Health Michigan also offers a Dual Eligible Special Needs (HMO D-SNP) plan, CCA Medicare Maximum, designed for those on both Medicare and Medicaid. These members can pay $0 for many services, including doctor visits, hospital stays, prescriptions, hearing and vision exams, and transportation to medical appointments. These members also receive extra benefits that can add up to $8,375 in healthcare costs savings each year, including comprehensive dental coverage and allowances for over-the-counter health products, groceries, sneakers, and utilities1.

CCA Health Michigan currently serves the following counties: Genesee, Lapeer, Macomb, Monroe, Oakland, St. Clair, and Wayne.
