First-of-its-kind program in the Ocean State designed to integrate the coordination of medical care and long-term services and supports
PROVIDENCE, R.I. – October 19 – CCA Health Rhode Island, a mission-driven healthcare services organization offering high-quality health plans and care delivery programs designed for individuals with significant needs, announced today a partnership with West Bay Community Action to launch an innovative program that will integrate the coordination of medical care and long term services and supports (LTSS). The partnership will increase communication between the two organizations to help deliver highly coordinated, person-centered care focused on improving clinical outcomes and member experience.
“Integrating medical services and social supports is foundational to CCA Health Rhode Island’s uncommon care® model, which is proven to help our members live safely in the community,” said Corey McCarty, Senior Vice President and General Manager, CCA Health Rhode Island. “We’re proud to launch this partnership with West Bay Community Action, and we are committed to continuing to proactively explore opportunities to bring innovative and impactful care delivery solutions to the Ocean State. Together, with our providers and community partners, we can make a real difference in the lives of those we serve.”
As part of the program, CCA Health Rhode Island’s care team and West Bay Community Action’s LTSS case managers will collaborate on care coordination activities to provide a seamless experience for shared members in Kent County that are receiving long-term services and supports from Medicaid.
“We are thrilled to partner closely with CCA Health Rhode Island to support our shared mission of helping individuals with significant needs live safely in their homes,” said Paul Salera, President and CEO, West Bay Community Action. “This partnership will help break down silos, remove administrative barriers, and ensure that each and every individual receives the care and support they need, when and where they need it.”
Headquartered in Providence’s Innovation and Design District, CCA Health Rhode Island has been active in the state since late 2020, building a strong network or providers and other community partners, with a focus on addressing the full spectrum of members’ needs—including the social factors that impact health.
