CCA Health Rhode Island has announced a first-of-its-kind partnership with West Bay Community Action to launch an innovative program that is designed to increase communication between the two organizations to coordinate medical care and long-term services and support.
“Integrating medical services and social supports is foundational to CCA Health Rhode Island’s uncommon care model, which is proven to help our members live safely in the community. We’re proud to launch this partnership with West Bay Community Action, and we are committed to continuing to proactively explore opportunities to bring innovative and impactful care delivery solutions to the Ocean State.
