Making sure that older adults get the right care—when they need it—requires not only the right team working toward the individual’s health goals, but also innovative technology that can predict their needs in real time. Tailoring healthcare to seniors’ needs should also consider the “Age-Friendly Health Systems” framework that addresses four elements: what matters, mentation, mobility, and medication. Dr. Wayne Saltsman, senior medical director of CCA Senior Care Options, discussed how CCA uses this framework, as well as information technology—such as predictive analytics that find patterns to help predict a member’s future needs—to meet seniors’ goals of care and prevent hospitalizations, ER visits, and more.
