A recent study published in the American Journal of Managed Care (AJMC) found that intensive care management was associated with a reduction in both total medical expenditure and in utilization. As part of a pilot program with MassHealth, Commonwealth Care Alliance provided intensive, community-based, multidisciplinary primary care for Mass General Brigham patients identified as having the highest cost and highest risk based on utilization and medical, social, and/or behavioral health complexity. Patients in the study who received intensive care management in the community/home setting and, in some cases, home-based primary care, experienced a reduction in emergency department visits and inpatient hospital admissions, as well as a lower per member per month total medical expense.
