If you have Medicare and Medicaid, you are eligible for CCA Health Michigan Medicare Maximum (HMO D-SNP). This plan provides all the coverage of Medicaid and Medicare, plus more benefits at no cost to you. See the chart below for benefit details.
Residents of Genesee, Lapeer, Macomb, Oakland, St. Clair, and Wayne Counties qualify for our Medicare Excel and Medicare Ultima plans.
If you are eligible for Medicare only, you qualify for CCA Health Michigan Medicare Excel (HMO) or CCA Health Michigan Medicare Ultima (HMO). Both of these plans provide “all-in-one” coverage at affordable monthly plan payments.

Our Medicare Advantage plans vary in price and in the benefits they offer. When choosing a plan, you should take into account two factors: what it covers and what it costs.
CCA Health Michigan Medicare Excel offers a wide range of benefits for $0 a month. When you sign up, you get all the coverage of Medicare. Plus, routine dental services, prescription drugs, help with over-the-counter products, and more.
CCA Health Michigan Medicare Ultima offers all the same benefits as CCA Health Michigan Medicare Excel. But it also includes additional benefits and greater savings for some services. In exchange, you pay a low monthly plan payment of $40 per month. If you qualify for the Extra Help program, you may even pay less.
If you qualify for a D-SNP plan, CCA Health Michigan Medicare Maximum offers the most robust benefits of all our plans. Benefits include all the coverage of Medicaid, Medicare, and more—for $0 a month.

Important Considerations
As we noted earlier, you should weigh cost vs. coverage and determine what is most important to you in a health plan. For instance, if you don’t want to pay a monthly premium, CCA Health Medicare Excel is a wise choice. If you want or need additional benefits, you may want to consider CCA Health Medicare Ultima. Also note: If you qualify for Extra Help, your monthly premium for the CCA Health Medicare Ultima plan may be lower.
