The CCA Quality Improvement Program
At Commonwealth Care Alliance® (CCA), we partner with you to develop programs that support your needs and preferences. This helps improve your health and well-being. It can also help promote your safety and independence.
The CCA quality improvement program aims to:
- Understand the needs and expectations of members and their caregivers.
- Improve member experience and support shared decision making.
- Use data and input from clinicians and administrators to improve care delivery.
- Identify and share best practices and clinical practice guidelines with clinicians.
- Support members in need of long-term services and supports.
The goal of the CCA quality improvement program is to optimize the:
- health of a population
- experience of care
- cost of care
- well-being and safety of our workforce
- equity of care
These are the Institute for Healthcare Improvement’s “Quintuple Aim”.1
CCA can achieve these goals by focusing on the “Six Domains of Healthcare Quality.”1
These were developed by the Agency for Healthcare Research and Quality (AHRQ).
- Safety: Member and patient injuries are avoided or reduced.
- Effectiveness: Overuse of ineffective and underuse of effective care and service are avoided.
- Timeliness: Care and services are delivered when they are needed. Excessive wait times are reduced when possible.
- Person-Centeredness: Care and services align with the individual’s needs and preferences.
- Efficiency: Processes are developed to avoid excess costs and waste of resources.
- Equity: Care and services do not vary based on people’s race, ethnicity, gender or culture.
How We Aim to Deliver Person-Centered Care
- Partner with members to develop care goals and plans.
- Empower members to make informed decisions about their care.
- Respect members’ values, preferences, and needs.
- Honor members’ dignity and independence.
- Reduce members’ fear and anxiety by creating easy to read information.
- Involve care teams, family, and friends (as desired by the member).
- Integrate medical and social supports and improve transitions of care.
- Focus on the psychological, social, and spiritual needs important to members.
- Help members improve their quality of life by:
- facilitating community integration, inclusion, and participation.
- supporting their ability to reach optimal health and function.
