Meet your CCA Care Coordinator

Your CCA Care Coordinator will call you to tell you more about using your new benefits. They will take an active role in making sure you receive the care you need, including scheduling appointments, talking with all your healthcare providers, and answering any questions you may have.

Have your initial visit with a nurse

Your Care Coordinator will schedule your visit with a registered nurse. The nurse will review your overall health and look for opportunities to help you handle day-to-day tasks. This visit is the first step toward developing a tailor-made care plan that will ensure you get the absolute most from your benefits.
