If you’re entitled to Medicare Part A (hospital insurance) and enrolled in Medicare Part B (medical insurance), we can help you choose the CCA Medicare plan that works best for you. CCA Health offers two Medicare Advantage plan options with the benefits you need to manage your health and lower your out-of-pocket costs. CCA Medicare Preferred (PPO) and CCA Medicare Value (PPO) plans give you a wide range of benefits. Based on the plan you choose, that includes:
CCA Medicare Preferred
- $01 monthly plan payment with $0 medical deductible
- $0 primary care copays
- Prescription drugs as low as $0
- $0 copay for preventative dental services2
- $0 routine eye exam (1 per year), $0 copay for base lenses and $400 annual allowance for eyewear
- $0 labs3
- $150 every calendar quarter to purchase plan-approved over-the-counter-items4
CCA Medicare Value
- $291 monthly plan payment with $0 medical deductible
- $0 primary care copays
- Prescription drugs as low as $0
- $0 copay for preventative dental services2
- $0 routine eye exam (1 per year), $0 copay for base lenses and $400 annual allowance for eyewear
- $0 labs3
- $160 every calendar quarter to purchase plan-approved over-the-counter-items4
PLUS! Members with certain qualifying chronic conditions receive a $100 annual allowance to purchase sneakers and can purchase food with their Healthy Savings card.5
