Prioritizing health related social needs
Since the inception of our health plans, health related social needs have been at the core of our nationally recognized care model, which prioritizes unmet social and behavioral health needs. Depending on each member’s needs, care plans may be individualized to address behavioral health, food and housing security, accessing community resources, crisis support, transportation, and more.
Nearly half of all CCA One Care (Medicare-Medicaid Plan) and CCA Senior Care Options (HMO D-SNP) members received our assistance with at least one social support in 2023
