Your MDS/comprehensive assessment is important

This assessment helps us understand you as a whole person—not just your medical needs. This way, we can create a care plan that fits your needs and helps you make the most of your health plan benefits.

A nurse will meet with you—either at your home, a place you choose, or by video—to complete the assessment. During the visit, you’ll be asked to share information such as:

This assessment is also a great time for you and your caregivers to ask questions about your care. We do this assessment once a year for our CCA One Care members and twice a year for our CCA Senior Care Options members to make sure we have the latest updates to support your care.
