We want to call your attention to the recurring roster update reminder located at the bottom of each issue of CCA Provider News. This serves as a standard reminder to keep your physician roster and practice information current with CCA, as it is critical that we have accurate records.
CCA requires that our contracted providers submit provider additions, deletions or demographic changes to us. This information not only ensures CCA members can find you in CCA’s provider directory, but also ensures that claims are paid accurately and in a timely manner.
Please take a moment to ensure we have current information from your practice, including your:
- Unique, 10-digit Organization National Provider Identifier (NPI) number
- Taxpayer Identification Number (TIN)
- Service location
- Practice phone number
- Fax number
To add a single practitioner, please complete the HCAS Provider Enrollment Form and submit it to the CCA Provider Data Management (PDM) department at [email protected]. To update more than ten practitioners at once, please reach out to the PDM department via the email address to receive your medical roster.
