30-year-old Lindsey, a CCA member since 2018, has experienced chronic housing insecurity and homelessness since she was a teenager. A long-time patient of Lynn Community Health Center (LCHC), Lindsey had worked with an LCHC long-term services coordinator who tirelessly helped her apply for housing waitlists year after year, in the hunt to secure permanent housing. 

In the spring of 2024, with the assistance of her LCHC care manager, Lindsey finally found an apartment in Lowell. Lindsey loved the apartment but needed financial assistance with the rent and security deposit. CCA connected Lindsey’s LCHC care manager with a Massachusetts program that could help Lindsey with financial support for her housing needs. After confirming with the building’s property manager that they would be willing to work with this program, Lindsey’s care manager helped her apply. Less than two months later, Lindsey was in her new apartment. 

“I’m so happy to be in an amazing place to call home,” Lindsey said. 

CCA and Lynn Community Health Center work closely together to care for CCA members in the Lynn area. Delegated provider partnerships like this – which extend CCA’s care model out to community-based organizations, like LCHC – ensure the success of our shared mission and give members like Lindsey the chance to reach their goals and improve their lives.
