Caregivers and support systems play an integral role. Engaged caregivers can help reduce unexpected hospital visits for those they support by up to 20 percent.
Family members and friends who care for loved ones at home are essential to their health and safety. But without the right support, caregivers often face stress, burnout and tough decisions – leading to avoidable hospitalizations, and earlier long-term care placement for those they support.
That’s why CCA has partnered with Trualta, an online caregiver education and support platform. Trualta gives caregivers the skills and confidence to provide safe, effective care at home.
The platform offers:
- Quick, on-demand lessons on daily care, medical tasks, and managing chronic conditions.
- Guidance on communication, planning, and end-of-life considerations.
- A library of resources on specialized topics including cancer, dementia, diabetes, and children with special needs.
- Tools to support caregiver well-being and resilience.
- Access to 1:1 coaching and online support groups.
Who’s eligible?
Trualta is available at no cost to family members or informal caregivers providing in-home support for CCA Senior Care Options and One Care members.
How you can help:
- Share cca.trualta.com with caregivers during in-person visits, phone calls, or follow-ups.
- Print the link or add it to discharge instructions, if appropriate.
- Encourage caregivers to use Trualta’s resources for support and education.
