You have or are eligible for both Medicare Part A (hospital insurance) and Medicare Part B (medical insurance)

You qualify for the Michigan State Medicaid program
If you don’t qualify for Medicaid, you may be interested in one of our CCA Medicare Advantage plans

You live in the CCA Medicare Maximum service area, which includes all cities and towns in Genesee, Lapeer, Macomb, Oakland, St. Clair, and Wayne Counties
Don’t have Medicaid?
We also offer CCA Medicare Advantage HMO plans with MORE benefits for those eligible for Medicare but NOT Medicaid.
CCA Medicare Maximum (HMO D-SNP) is a health plan with a Medicare contract and a contract with the State Medicaid program. Enrollment depends on contract renewal.
