Our Service Area
Our CCA Medicare Advantage plans are available in the following counties: Genesee, Lapeer, Macomb, Monroe, Oakland, St. Clair, and Wayne.
Our CCA Medicare Dual Special Needs plan is available in the following Michigan counties: Genesee, Lapeer, Macomb, Oakland, St. Clair, and Wayne.
Please note that the names of our plans will be changing for 2023:
- Cardinal Plan (HMO) will be called CCA Medicare Ultima
- Principle Plan (HMO) will be called CCA Medicare Excel
- Dual Care Plus Plan (HMO D-SNP) will be called CCA Medicare Maximum
