Our Service Area
The Principle (HMO) plan and the Cardinal (HMO) plan are available in the following counties: Genesee, Lapeer, Macomb, Monroe, Oakland, St. Clair, and Wayne.
The Dual Care Plus (HMO D-SNP) plan is available in the following Michigan counties: Genesee, Lapeer, Macomb, Oakland, St. Clair, and Wayne.

*In Monroe County: The Principle plan and the Cardinal plan are available; the Dual Care Plus plan is not yet available.
The Principle (HMO) and the Cardinal (HMO) plans are health plans with Medicare contracts. Enrollment depends on contract renewal. The Dual Care Plus (HMO D-SNP) plan is a health plan with a Medicare contract and a contract with the State Medicaid program. Enrollment depends on contract renewal.
