A focus on complex care for high-need individuals

At the core of our mission is our comprehensive model of uncommon care. This unique model delivers better health outcomes through seamless integration, a community focus, and an emphasis on the behavioral and social determinants of health that account for so many critical gaps in care. It is based on our deep understanding of what puts people at risk, together with our unmatched ability to find and engage hard-to-reach individuals.

The rewards of uncommon care go beyond improved clinical and financial performance. Member and patient satisfaction scores also improve significantly: for the past five consecutive years the CCA Medicare-Medicaid plan has earned a top rating in CMS’s nationwide consumer survey, scoring 93% in 2020.1

Prioritizing social determinants of health

Since the inception of our health plans, social determinants of health (SDOH) have been at the core of our nationally recognized care model, which prioritizes unmet social and behavioral health needs. Depending on each member’s needs, care plans may be individualized to address behavioral health, food and housing security, accessing community resources, crisis support, transportation, and more.
