Effective January 1, 2026, the One Care program will transition from a Medicare-Medicaid Plan (MMP) to a Fully Integrated Dual Eligible Special Needs Plan (FIDE SNP). This change means eligible individuals will enroll directly with managed care organizations that participate in the One Care program rather than through the state.
As the enrollment process transitions for One Care Plans, CCA will:
- Accept referrals directly from provider and other community organizations
- Support individuals in each step of the enrollment process
- Ensure individuals are fully informed about the One Care benefits and care coordination support they can rely upon
