When James first joined CCA Health, his mental health was suffering. With no family or other social support nearby, he had frequent hospitalizations. His CCA Health care partner, Shannon, an R.N., tried many times to reach James. After months of persistent outreach, she was able to earn his trust.

During her first few meetings with James, Shannon completed a comprehensive evaluation of his health. While his mental health hospitalizations were concerning, James also needed help managing his diabetes, taking his medications, and keeping up with doctor visits. On top of all of that, he was on the brink of being evicted from his home.

To help, Shannon visited James in his home and developed a care plan just for him. First, she coordinated with his mental health providers to ensure his medications could be delivered to his home. Based on his medical needs, she also helped him schedule necessary appointments, first with a primary care physician and then with various specialists.

Shannon attended some of these appointments with James and helped to schedule follow-up tests. This level of support proved critical when one of his medical tests uncovered a new issue that required surgery.

For James, personalized care has made a difference in meeting his health goals. With Shannon’s support over the past year and a half, he has made great progress. His experience working with Shannon has improved his physical and mental health, allowing him to get back to hobbies he enjoys. Now that his health is more stable, he likes to go for walks around his neighborhood.

By delivering patient-centric care that integrates his medical, mental health, and social support needs, Shannon has earned his trust and gratitude.
