Ensuring CCA members receive timely preventive care, including bone health assessments, is not only an important HEDIS quality measure, but is also critically important to the long-term health and wellness of those we serve.
That’s why CCA has partnered with Transtreme to offer in-home osteoporosis screenings for eligible members* to help reduce the risk of future fractures and support better long-term outcomes. These visits are:
- Provided at no cost
- Does not affect coverage
- Designed to complement, not replace the care you provide
These screenings offer additional support by identifying at-risk patients and encouraging them to maintain preventive and routine care with you. Transtreme will contact eligible members directly to schedule an in-home visit. Results will be faxed to your office for your review and follow-up as needed.

*Eligible Members: Female members age 67–85 with a fracture, no bone mineral density (BMD) test in the past 24 months, and no osteoporosis medication in the past 12 months. Screenings must be completed within six months of the fracture date.
