Partnering with you to keep your patients living safely and independently in their homes and communities is at the heart of our mission. Our nationally recognized care model is proven effective in addressing unmet social determinants of health, behavioral health, and medical needs. We help you extend your reach into your patients’ homes and communities through care coordination, behavioral health, and long-term services.

CCA’s uncommon care® model has demonstrated success in building care partnerships with CCA One Care members and improving care for people with significant needs, while also lowering the long-term costs of care by reducing avoidable hospital re-admissions and institutional care.

Delivering uncommon care® during a crisis

From the very first warnings of the oncoming crisis, CCA took swift and bold actions to meet unprecedented challenges and adapt to new realities. We aligned closely with our provider partners to help meet the medical, behavioral, and social needs of our members and patients—helping to minimize gaps in care during challenging times.
