NEW: CCA Standardized Prior Authorization Form

CCA has a new Standardized Prior Authorization form to ensure that minimal processing information is captured. An attestation was added as a certification that any request submitted with the expedited timeframe meets the CMS criteria. 

The new form is now available for download on the CCA website. Filling out this form completely and accurately will help streamline the prior authorization submission process.

We are committed to providing timely services to our members. We continuously strive to improve on provider experience by improving our processes, and we ask that providers take the time to accurately complete authorization request forms to ensure timely turnaround for service requests.
