You’re eligible for CCA One Care (HMO D-SNP) if:

• You are between the ages of 21 and 64
  Age 65 or over? You may be interested in our CCA Senior Care Options Plan.

• You are eligible for MassHealth Standard or MassHealth CommonHealth
  

• You are entitled to Medicare Part A (hospital insurance), you are enrolled in Medicare Part B (medical insurance) and qualify for Medicare Part D (drug coverage). 

• You live in our service area, which includes all cities and towns in the following counties of Massachusetts: Barnstable, Berkshire, Bristol, Essex, Franklin, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester
