Programs that cater to your unique needs
As a Commonwealth Care Alliance (CCA) member, you have access to all sorts of programs to support your health and well-being. Whether it’s connecting you with other CCA members, helping you understand your health better, or providing a unique level of support beyond your standard CCA benefits, these programs are here to help! They are available at no additional cost to members who need them.
To develop your care plan, your CCA care team may ask you questions about your health. This also helps us develop special programs designed to keep you healthy and well.

Caregiver Support
CCA offers help to our members’ caregivers to manage the day-to-day challenges that they face. We offer support and resources so caregivers are best equipped to assist with a CCA member’s care while managing their own health and well-being. CCA Nurse Advice Line
Get urgent care support 24 hours a day, 7 days a week. If you experience an urgent health concern, like a mild fever, vomiting, or shortness of breath, call 866-610-2273 (TTY 711) and press 3 to speak to a registered nurse or clinician.
CCA Primary Care
CCA Primary Care is a welcoming place where you will receive all the high-quality, personalized care and services you need. As a CCA health plan member, you know firsthand our dedication to your health and well-being. Complex Care Self-Management Program
The CCA Complex Care Self-Management program supports our members with ongoing conditions such as heart failure, COPD, and diabetes. A trained healthcare professional will work with your primary care provider to provide you services and tools that help you take control of your condition. The program helps CCA members stay healthier, avoid unnecessary hospital visits, and develop a plan to feel better! Doula Program
CCA offers doula services to support our members during pregnancy, labor, birth, and postpartum (after childbirth). CCA doulas receive special training to provide the physical and emotional support you need. They can also provide helpful information so you and your loved ones can make informed choices about birth and your baby’s care. High Intensity Care Management (HICM)*
Members in this program** have conditions like diabetes, kidney disease, and heart disease. They may also have substance use and mental health needs. Your HICM team includes a Nurse Practitioner or Physician Assistant. They will work with your providers to develop a treatment plan, which may include:
- Medical and mental health care
- Medication support
- Guidance on nutrition and exercise
Your HICM team will support you through illness, recovery, and stability.
In-Home Urgent Care
Through the instED® program, CCA can send a paramedic to your home when you need urgent care. The program can help you avoid the emergency department or urgent care clinic when it’s not medically necessary. The instED program is available in select areas. Learn more about instED.
Member Voices
Member Voices brings our members together to share their experiences about the care they receive from CCA and its provider partners. We use your input to design and improve the services and programs we provide. It’s also a great way to connect with other CCA members and learn from their experiences. Mental and Behavioral Health
These services** offer support for conditions that affect your thoughts, emotions, and behaviors. These may include schizophrenia, anxiety, or depression. You can also get support for substance use disorder. Your CCA care team can help you with a medication plan, therapies, and more.
Palliative Care
The CCA palliative care program is designed to improve the whole health of our members with serious illnesses. Our team of experts partner with you to help you and your caregivers make plans according to your wishes and beliefs. Learn more about our palliative care program.
Pharmacy Programs
CCA offers support with your medications through a range of programs, including mail order, specialty medications, extended supply, med sync, and medication therapy management. Learn more about our pharmacy programs.
Preventive Care
This program sends you reminders about important screenings and vaccines. One example is our flu vaccine program. It helps remind you of the importance of getting a flu vaccine each year and where to get one.
Substance Use Program
The CCA substance use program ensures that members who are experiencing or are at risk for developing opioid use disorder or any other substance use disorder can get the care they need. In addition to providing coverage for certain addiction services, CCA provides targeted clinical support. Our network of providers and the benefits we offer provide services such as detoxification, intensive outpatient treatment, acupuncture, and medication-assisted treatments. CCA also offers detoxification with behavioral health support for some members. Transitions of Care*
This program helps you safely move back to your home after leaving the hospital. Your CCA care team checks in on you and connects you to the services you need to recover. They can also help schedule all your follow-up appointments.
Virtual Care
You can get certain care from CCA without leaving your home. Services like annual wellness checks, routine and urgent visits, screenings, behavioral health support, and more can be provided over a video call.
