You’re eligible for CCA Senior Care Options (HMO D-SNP) if:

    You are 65 years or older
    Under the age of 65? You may be interested in our CCA One Care (Medicare-Medicaid) Plan.
  • 2

    You are eligible for MassHealth Standard and Medicare, and you do not have any other comprehensive health insurance

    Note: Beginning January 1, 2026, all SCO enrollees must be enrolled in both Medicare Parts A and B, as well as MassHealth Standard (Medicaid), to remain in a SCO plan. For more information on this change, please visit: 2026 SCO Eligibility Changes | Mass.gov1

    You live in the CCA Senior Care Options service area, which includes all cities and towns in Bristol, Essex, Franklin, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester Counties
