The CCA Quality Improvement Program Aims To:

The Goal of the CCA Quality Improvement Program is to Optimize the:

  1. Health Outcomes.
  2. Experience of care.
  3. Cost of care.
  4. Workforce well-being and safety.
  5. Equity in care.

These are the Institute for Healthcare Improvement’s “Quintuple Aim”.1

CCA works toward these goals by focusing on the “Six Domains of Healthcare Quality.”¹ These domains were developed by the Agency for Healthcare Research and Quality (AHRQ).

  1. Safety: Prevent or reduce member and patient injuries.
  2. Effectiveness: Avoid ineffective care and increase the use of effective care and services.
  3. Timeliness: Provide care and services when they are needed and reduce wait times when possible.
  4. Person-Centeredness: Deliver care and services that reflect each member’s needs and preferences.
  5. Efficiency: Reduce unnecessary costs and waste of resources.
  6. Equity: Provide high-quality care and services regardless of race, ethnicity, gender, or culture.

How We Aim to Deliver Person-Centered Care

  1. Partner with members to develop care goals and care plans.
  2. Support members in making informed decisions about their care.
  3. Respect members’ values, preferences, and needs.
  4. Honor members’ dignity and independence.
  5. Provide easy-to-read information to help reduce fear and anxiety.
  6. Involve care teams, family, and friends when the member chooses.
  7. Coordinate medical care and social supports and improve transitions between care settings.
  8. Address members’ psychological, social, and spiritual needs.
  9. Help improve quality of life by:
    • Supporting community involvement and inclusion.
    • Helping members achieve their best possible health and function.
