Frail Elder Waiver Provider Form
To meet State regulations and maintain program integrity, all contracted providers delivering Frail Elder Waiver (FEW) services to CCA members must complete the CCA Frail Elder Waiver Audit Form. This process ensures critical standards:
- Qualified Providers: The State requires that all Senior Care Options plans ensure FEW participants are able to access waiver services from competent and qualified service providers. Completing this form enables CCA to perform critical oversight of network providers delivering FEW services, ensuring all regulatory requirements to provide care under the FEW program are met.
- Health and Welfare Assurance: The State mandates an effective system to safeguard the health and welfare of waiver participants. Accurate and timely submission of this form supports oversight, reduces risk, and ensures participants receive safe, appropriate services.
- Compliance and Documentation: The State is required to report quality data annually on all waiver service providers to the Centers for Medicare and Medicaid Services (CMS). All SCO plans are required to submit our compliance oversight tracking to the Executive Office of Health and Human Services (EOHHS) to support the State’s annual reporting requirements.
Submitting the Commonwealth Care Alliance (CCA) FEW Audit Form is required for compliance, preventing service delays, and maintaining high standards of care. Please review the FEW Provider Qualifications documents below prior to completing the CCA FEW Audit Form. In addition to completing the CCA FEW Audit Form, you are required to send the form as well as a copy of your agency-level license(s) and/or certification(s), if applicable, to [email protected].
