CCA Model of Care Training

The Centers for Medicare & Medicaid Services (CMS) requires all providers to complete an annual Commonwealth Care Alliance (CCA) Model of Care (MOC) training if you are either contracted to see CCA SCO or One Care members or are an out-of-network licensed independent practitioner (LIP) who routinely treats and/or provides services to CCA SCO or One Care members. The intent of the CMS requirement is to ensure providers can seamlessly coordinate the care for this population with complex needs.

The following training steps must be completed no later than 12/31/2026.
