About Commonwealth Community Care (CCC)

Commonwealth Community Care (CCC) is the clinical affiliate of Commonwealth Care Alliance with specialized primary care centers located in Boston, Lawrence, and Springfield as well as an office location in MetroWest/Worcester. The three primary care center locations provide comprehensive, disability-competent care to adults of all ages, including those who may have complex physical, developmental, intellectual, and behavioral health conditions. The design of these care centers is based on CCC’s 30 years of prior experience developing unique facilities and programs to serve individuals with disabilities. The MetroWest/Worcester office location coordinates home-based care services.

We’ve learned that to provide excellent care, we need to take the time to understand and respect the self-knowledge of our patients. Our staff are trained in the unique needs of people with complex health conditions, including people with disabilities. We're committed to the principles of consumer direction, independence, and self-determination.


Care where patients need it

Depending on each patient's needs and preferences, we can provide care in the home or in one of our specially equipped, accessible care practices. Staff are available 24/7 and coordinate care as needed with the patient's preferred services and providers, as well as with the people in their life.

Our unique approach to care means that patient's have access to a team of healthcare professionals: primary care physicians, nurse practitioners, physician assistants, social workers, physical/occupational therapists, and specialists in behavioral healthcare services. Team members collaborate closely to coordinate care based on each patient's individual needs. Our administrative staff provides assistance with transportation and referral arrangements, and assist with needs for equipment and supplies.


The right care at the right time

CCC clinicians are trained in end-of-life care, and are skilled at supporting patients, families and caregivers through symptom management and access to community resources. Our goal is to help you stay in the community – and in the care of the people you know and trust – through the end of life, should that be your choice. We have a close partnership with CCA’s palliative care team, which consists of doctors and registered nurses. We also partner with traditional hospice agencies for additional services to augment primary care, or for full services when required.
