CCA Medicare Advantage Plans

If you’re eligible for Medicare, CCA Health offers two Medicare Advantage plans with a wide range of benefits. These include things like prescription drug coverage, $0 primary care visits, and $0 annual dental and eye exams.

CCA Medicare Special Needs Plan

If you’re eligible for Medicare and Medicaid, you may qualify for our Special Needs Plan, CCA Medicare Maximum (HMO D-SNP). Our plan connects you to a coordinated community of care—at $0 cost to you.1 That includes prescriptions, dental services, and $750 every calendar quarter for over-the-counter health products.2

CCA Medicare Preferred (PPO) and CCA Medicare Value (PPO) are health plans with a Medicare contract. Enrollment depends on contract renewal. 

CCA Medicare Maximum (HMO D-SNP) is a health plan with a Medicare contract and a contract with the State Medicaid program. Enrollment depends on contract renewal. 

Support and services are based on the health plan selected and on individual member needs.
