What is the Healthy Savings card?
The Healthy Savings card is a prepaid Mastercard that gives you access to quarterly funds that can be used to pay for CCA-covered items. CCA partners with NationsBenefits® to make these benefits easy and convenient for you to access.
Do I get a Healthy Savings card with my plan?
All CCA Senior Care Options (HMO D-SNP) plan members receive a Healthy Savings card.

How does the Healthy Savings card work?
- Every quarter on January 1st, April 1st, July 1st, October 1st, your Healthy Savings card funds are reloaded automatically. If you enrolled in the middle of a quarter, you will have access to your funds upon enrollment.
- You can use your card to pay for CCA-covered items using your quarterly allowance. As you make purchases with your Healthy Savings card, costs will be deducted from your available balance.
- Funds don’t carry over from quarter to quarter. To get the maximum benefit, make sure you use the full balance before the end of each quarter.
- You can register for the Benefits ProTM Portal to check your card balance, check item eligibility, place an order online, access store locator and more. To register visit CCA.NationsBenefits.com2 or download the Benefits ProTM App using this QR code to register.
- You also can check your balances by calling the number on the back of your Healthy Savings card.
- Your card cannot be used at an ATM or for cash back when making a purchase. No PIN or signature is required when using your card at a store. If a PIN is requested, just press enter or select the credit option.

It is easy and convenient for you to use these benefits. Whether you prefer to purchase products in-person or from the comfort of your home, CCA offers a variety of ways to use your Healthy Savings card.
Home Delivery:
Shop and have your products delivered to your doorstep free of charge. To do this, you can choose one of the following options:
- By phone: Simply call NationsBenefits at 833-891-6886 (TTY: 711), 8 am to 8 pm, 7 days a week
- Mail-order: Use the Mail Order form
- Online: You can access the Benefits ProTM Portal by clicking here or visiting CCA.NationsBenefits.com2

The Healthy Savings card can be used at many retailers including: CVS, Dollar General, Price Rite, Save a Lot, Shaw’s, Star Market, Stop & Shop, Walgreens, Walmart, and more.

Healthy Savings Card Common Questions
Call the phone number on the activation label on the front of the unused card to activate the Healthy Savings card before you use it. You will need your CCA member ID number, which is listed on your CCA member ID card.
On the first day of each quarter (January 1st, April 1st, July 1st, October 1st) your Healthy Savings card will automatically reload. You have the entire quarter to use the balance amount. Any balance remaining at the end of the quarter will not roll over. If you do not use the full amount, you will lose the remaining balance.
You can use your card to buy CCA-covered over-the-counter items listed in the CCA catalog. You do not need a prescription to buy these products. Certain brands may not be carried by all participating retailers. Any category not listed is not covered; examples include candy, razors, and cosmetics.
If you have a documented, active, qualifying, chronic condition, you may also use your Healthy Savings card to cover CCA-covered food items and utilities3. For a listing of these items, you can visit the CCA catalog. Learn more about eligibility for additional benefits on our SSBCI webpage.

No. Your Healthy Savings card cannot be used as a form of payment for prescriptions. Always use your CCA member ID card to buy your prescriptions. If you have a prescription for an over-the-counter item, use your CCA member ID card and fill your prescription as you normally would.
If you forget to bring your Healthy Savings card with you to the store, you can still buy the eligible products that you need and submit a request for reimbursement within one year of your purchase. To request reimbursement, print a copy of the CCA Reimbursement form, and follow the instructions on the form. Mail your completed paper reimbursement form to CCA Member Services with a copy of the receipt(s) for the covered items you bought. If approved, we will reimburse you for the covered item(s) and take off that amount from your current Healthy Savings card balance.
You must treat your card like cash. If a card is lost or stolen, please notify us right away by calling CCA Member Services at 866-610-2273 (TTY 711), 8 am to 8 pm, 7 days a week. You can also request a new card at CCA.NatonsBenefits.com2, or by using the Benefits ProTM App. We will send you a replacement card. Any balance you had remaining for that quarter will be loaded onto your replacement card. Dollars spent without your authorization will not be replaced.
