First-of-its-kind program in the Ocean State designed to integrate the coordination of medical care and long-term services and supports

PROVIDENCE, R.I. – October 19CCA Health Rhode Island, a mission-driven healthcare services organization offering high-quality health plans and care delivery programs designed for individuals with significant needs, announced today a partnership with West Bay Community Action to launch an innovative program that will integrate the coordination of medical care and long term services and supports (LTSS). The partnership will increase communication between the two organizations to help deliver highly coordinated, person-centered care focused on improving clinical outcomes and member experience.

Together, with our providers and community partners, we can make a real difference in the lives of those we serve.”

As part of the program, CCA Health Rhode Island’s care team and West Bay Community Action’s LTSS case managers will collaborate on care coordination activities to provide a seamless experience for shared members in Kent County that are receiving long-term services and supports from Medicaid.
