CCA’s Community Health Workers play an integral role in a member’s care team. They work to connect members with the community resources, social programs, and healthcare providers they need to stay healthy. These resources can include transportation, food delivery programs, connecting them with an eviction lawyer, or helping them apply for rent assistance programs.
