By 2030, the number of adults over 65 who experience homelessness is expected to triple. Permanent supportive housing―a model that combines long-term housing, health care, and supportive services―has shown promising results in reducing homelessness and decreasing emergency department visits and hospitalizations.

This blog post details a partnership between Commonwealth Care Alliance and Hearth, a non-profit community-based organization focused on ending elder homelessness, to provide permanent supportive housing for eligible individuals. It details the interdisciplinary team structure, program financing, and outcomes of this successful partnership to care for older adults experiencing homelessness.
