As of Q1 2023, Commonwealth Care Alliance (CCA) has enhanced its member-centered care planning to Medicare Maximum members in Rhode Island and those covered in Massachusetts by our Senior Care Options and One Care plans.
These CCA members will have an opportunity to review their care plan annually with interdisciplinary care members of their choice. Team members might include their care partner, community nurse, health worker, behavioral health specialist, geriatric support coordinator or long-term services support coordinator, and primary care provider. They can also include support from the member’s home environment, such as family or friend caregivers, or a personal care assistant. Each CCA member can choose whether the meeting is held by phone, virtually, or in person.
